Crossroads Ceredigion

Ceredigion Crossroads provides a professionally trained Carer Support Worker to stay for a few hours now on a regular or occasional basis, or perhaps overnight. The high quality service provided supports the carer and those with care needs, in their own homes. The person with care needs may be ill, disabled, or has mental health problems or with a life limiting illness. Our service is free of charge within the home and as a registered charity we are funded by grants from statutory agencies, fund giving Trusts, and donations or fund raising events.

Ty Cymorth (Day care centre/Carmarthen)

Highly trained palliative care health professionals provide a service for those whose illness allows them to remain at home but gives them the added benefit of receiving any treatment or advice which is needed to alleviate symptoms, in comfortable and friendly surroundings This can take the form of all aspects of support - physical, emotional , spiritual. Meeting with others in similar circumstances can be of great assistance to some and having the opportunity to talk to those who understand and be listened to is one of the ways they are helped.

In Care

The in-patient palliative care unit, works to relieve symptoms such as nausea and vomiting and pain. The usual stay for control of symptoms is approximately 10 to 14 days, using a holistic approach i.e. physical, psychological and emotional support. The unit comprises of six individual rooms with en suite, each with their own fridge, TV and video facilities. A phone is available in each room for incoming calls only (there is a personal phone number made available for each patient).

The Paul Satori Foundation offer a hospice home Care Service with a  team of around 40 nurses and carers around Pembrokeshire. Assistance is tailored to each individual’s needs and includes support to those who care for the patient. They provide an aromatherapy and equipment loan service.
